Individualized Family Service

How we can help


Our service is available to families raising children (ages 2-21) with any condition that is affecting the Brain and it's function. 

​Individualized Family Service (IFS) is a type of case management that looks at the entire needs of the child and family.  We explore the following areas: Therapeutic, Medical, Educational, Recreation, Family Needs, Parent Self Care, Crisis Planning and Transition into Adulthood Planning.  Families are given materials and Action Plans to address all of the concerns with resolutions. ​
Included in this Service:

  • Discovery, communication and Referrals with APPROPRIATE Mental Health and Medical Providers.

  • Educational Needs IEP/504/BIP and Homebound Meetings- We do require a 30 day notice for these meetings, so that we can review concerns and have a pre-meeting plan for best outcomes.

  • Recreation and community resources that will be a good fit for all members of your family.

  • Support and tools for families to function successfully, despite the challenges associated with Brain Disorders.

  • Crisis planning that is realistic and reduces the stress and trauma.
  • Complex conditions that require specialized treatments; including out of home placements, emergency Medicaid applications and long term care.

  • Transition into Adulthood starts at age 14 and continues until age 21. Direction and referrals with respect to what is important for your family.
