Two simple steps to get started:
There is no obligation or fees with getting started; this is an opportunity to learn more about how we can help and if this is the right service for your family.
The first meeting will be with our Intake Specialist, we can do over the phone or a virtual meeting. There is no cost or obligation. It takes about one hour.
Next step is to meet with a Navigator, this will be where we collect specific information about your child and family. Our first objective is to move your family out of crisis or immediate concern, with a comprehensive plan and resources. Together, we develop an Action Plan that is realistic and achievable.
Our consultation takes about 2 hours and is typically held with parents only. We can do this in our Concord Office, by phone or Zoom. Whatever works best for you!
After you receive your Action Plan, you have the choice to continue services with your Navigator or self advocate from the recommendations given.
Most families continue working with their Navigator and utilize about 2-3 Hours per month initially with a gradual reduction to 1 hour monthly check-in and then every few months another check in to work on future planning.
We attend IEP/School Related Meetings with you and continue support until the child transitions into adult services.
Individualized Family Service has served over 100 Families during its five years of existence.
We have helped children and families stop the cycle of hospitalizations, school suspensions and stays in facilities.
Helped complex children receive the Innovations Waiver or find placement in residential settings to bring appropriate care and peace for the child and family.
Whatever level of care and support your child and family needs, we will meet you in that place with understanding and passion
We are always seeking donations and writing grants to support this program in order to keep the cost low or free.
There is an easy application process that is explained in the Family Orientation for sliding scale discounts.
Hourly Service $25.00
Cabarrus County Residents will receive grant funding through the Cabarrus Recovery Grant. This project is being supported, in whole or in part by federal award number, SLFRP0347 awarded to Cabarrus County, NC by the US Dept of Treasury.
Programs for Specific Needs of Families
Project Baby Reach supports families with babies in the NICU at Atrium Cabarrus with support and resources.
This is no cost for families
Triple P is multiple session program for parents that is currently offered via Zoom.
Most parents complete the program in 4 one hour sessions that occur over 6 weeks.
There is no cost for Triple P. We have a grant from the Cabarrus Health Alliance to cover these services.
Planning for adulthood should begin at age 14 and is crucial for older teens. This program helps parents become educated about adult programs and options, we help you gather necessary documents, prepare you for steps to take to have access to adult benefits, services and supports for your child.
We have Interns from several Universities that work directly with your child to explore their interests and create a plan for the exit of high school.
